Work Permits for Summer Break 2024

Go to  https://www.hartdistrict.org/apps/pages/students-work-permits to apply for a work permit during Summer Break, or follow the steps below:

 

STEPS TO OBTAIN A WORK PERMIT

Please be sure you have completed the following steps prior to applying for a work permit:

 

STEP 1: Secure Employment

Students need to secure a job before obtaining a work permit.

STEP 2: Download the Work Permit Application

Click on the following link to download: 

• STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT–CERTIFICATE OF AGE CDE Form B1-1 (Rev. 02-14)

STEP 3: Complete the Work Permit Application:

• Students are to complete the sections “Minor’s Information” and “School Information.”

• Employers are to complete the section “For Employer to Complete” then print and sign their name.

• Parents/guardians are required to sign the form for knowledge and consent.

STEP 4: Email [email protected] that you are requesting a work permit. The auto-reply message will give instructions on how to set up an appointment. 

 

Work permits will be issued at the district office by appointment only during SummerSpring Break Monday, June 3 to Thursday, August 8. No permits will be issued on July 4. Work permits will be issued at Bowman starting Monday, August 12.on the following dates:
